Passport and Visa Requirements

 

Entry into a foreign country for official purposes may require a no-fee (official) passport and visa.  The DoD Foreign Clearance Guide (FCG) informs DoD personnel of such requirements.  While below may prescribe general procedures, it is important to understand that all actions must be done in accordance with the local DoD Passport and Visa Acceptance Agent processing the request for a no-fee passport or visa.  The DoD Passport and Visa Acceptance Agent's information requires you to log-in to see below.

 

Birth certificates must be a full transcript birth certificate. 

 

Orders showing a country that will require the issuance of a no-fee passport must be submitted as part of the application package.  If there are no orders, a justification memo may be used to demonstrate the need for a no-fee passport.  The memo must be in the same format and signed by the person noted at the bottom of the sample.

 

Ensure that your passport photo meets the requirements as annotated on the attached passport photo requirement sheet, or I will be unable to accept them.  I have attached a listing of 3 recommended places to get photos, please only use these as they are the only ones that I have never had problems with the passport photo's.  The Photos cannot have been used in any other passport.  Each passport has to use new photos. The background for the passport photo must be plain white, no shadows or off colors. Please follow all the instructions below, if there are any errors in the paperwork, It will handed it back to you and you will have to reschedule on another day to bring the items back in, once you have corrected the items.

 

A)  For applicants 18 and older, that have already had a passport issued to them before (either a tourist passport they applied for out in town or a prior No-Fee (Diplomatic, Official or No-Fee) passport), Use the below form:

 

Go to www.travel.state.gov

 

Follow the instructions there on the web site to complete a DS 82 form. Once you have completed it online (do not print out a blank form and fill it out in a typewriter or fill it out by hand as I will be unable to accept that), print it out from online. When you come to a screen that talks about money or paying for the passport mark the box passport book, click submit and on the next page look for create the document, then print out the DS82. If you are not going to send the passport in with the application, you cannot use this form. Do not sign.

Birth evidence: This is the only birth evidence used for this form Current/expired/expiring passport (It will have to go with the application)

 

B)  For children under 16 years of age (and children 16 and 17 as they have to have a parent sign the application), and first time applicants use the below form:

 

Go to www.travel.state.gov

 

Follow the instructions there on the web site to complete a DS 11 form. Once you have completed it online (do not print out a blank  form and fill it out in a typewriter or fill it out by hand as I  will be unable to accept that), print it out from online (do not  print out a blank  form and put it in  a typewriter or fill it out by hand because I will not be able to accept it). When putting the parents information, ensure you put the mother's maiden name Ensure that when you print it out, it is a DS-11, no other form will work. If asked if you have ever had a passport before when  completing the application online, the answer will be no so that the  correct form is utilized. When you come to a  screen that talks about money or paying for the passport click passport  book, then submit and then create form and print.

 

Birth evidence for Adult (all birth documentation will go with the applications, you will get them back, any one of the following three items will work): If you have a current no-fee passport, that will be your birth evidence.

1) Old passport not over 15 years in age ( or old no-fee passport if applicable) current passport will also work

2) Original Birth Certificate

3) Certified copy of birth certificate from State Registers Office of the State person was born in.

 

Birth evidence for Children (all birth documentation will go with the applications, you will get them back, any one of the following two items will work, plus if there is an old no-fee passport, that will have to be submitted with the birth certificate):

1) Original Birth Certificate

2) Certified copy of birth certificate from State Registers Office  of the State person was born in.

 

If Naturalized citizen:

1) Passport (US)

2) Naturalization Certificate (Original)

 

If Birth in Foreign Country (of US Citizen):

1) Will need original Embassy Birth Abroad document.

2) Passport (US)

 

C)  Both application types will need the following:

 

DD Form 1056.  PDF fillable Form attached. Block by block completion instructions attached (please follow the attached instructions when completing the form). Leave block 13 and 16 blank.  Do not get AO signature.

 

Clear copy of CAC Card  front/back and copy of spouse's dependent ID. Will also need copies of dependent ID for any children 10 years old and older (front/back)

 

Attached is the requirements for the photo, your photos must be just like the description on the attached. 1 passport photo taken out in town. There is nowhere on base to get passport photos. Do not attach to the passport application I will do that. If the photo does not match the attached

requirements, they will be rejected and you will have to go get new ones. I have attached a listing of 3 places to have your passport photo taken.

 

Appointments are 0900 thru 1100 or 1200 thru 1500 Monday thru Friday once you have completed all the above, send an e-mail for an appointment. You must have everything completed before the appointment (to include any copies that need to be done). Everyone must be at the appointment.

 

 

 

Visa application requirements vary by country.  Please contact the DoD Passport and Visa Acceptance Agent for the most current requirements for the country a visa is required.
