Staff Admissions Form

 

All NPS Federal employees are required to fill out the attached Staff Forms in addition to the online application when applying to a degree program. Please submit completed forms to admissions@nps.edu.

 

Participation Agreement Form

All DL applicants must submit the attached Participation Agreement concurrently with the online application. Please submit the completed form to admissions@nps.edu.

 

Navy Second Funded Degree Waiver Request Forms

If you are a USN officer and have already received a Navy funded degree (i.e. TA, NWC, NPS, and FSEP) and are interested in pursuing a second funded degree at the same level, you must request a waiver.  Please use the following templates to initiate the waiver.  If one has been granted, submit all completed forms to admissions@nps.edu.
